From (うそ) (uso, lie) + ()いたら (tsuitara, conditional of () (tsuku, to say)) + (はり) (hari, sewing needle) + 千本(せんぼん) (senbon, one thousand) + ()ます (nomasu, causative of () (nomu, to swallow)). Literally, whoever lies will be made to swallow a thousand needles.
